The Fusion Dance: Gogeta is not Vegito
Gogeta is the fusion of Goku and Vegeta that appears when they correctly perform the Fusion Dance, a technique invented by the Metamoran people; Goku learned it from them while he was in Other World. His name is a portmanteau: the “Go” from Goku followed by the “geta” from Vegeta.
To pull off the fusion, the two dancers must strike the poses in perfect symmetry with each other and bring their power to the same level: it's up to the stronger one to adjust to the other. The resulting fusion lasts about thirty minutes, depending on how much energy is spent.
Vegito is Goku and Vegeta's other great fusion: it's achieved with the Potara earrings, not the dance. The Potara fusion also blends their clothes, whereas the dance dresses the warrior in the Metamoran outfit: that's what gives Gogeta his vest, which Vegito doesn't have. Official publications don't agree on which fusion is the strongest.
Gogeta has Goku's tall build and Vegeta's stern features; in his base form, his hair is jet black. His short vest is black, with orange padding in the movie Fusion Reborn and in GT, and yellow padding in Dragon Ball Super: Broly, and he wears loose white pants. Like most fusions, he speaks with a dual voice: Goku's and Vegeta's, layered on top of each other.

1995: Gogeta faces Janemba in the movie Fusion Reborn
Dragon Ball Z: Fusion Reborn, the twelfth Dragon Ball Z movie, was released in Japan on March 4, 1995, during the Toei Anime Fair, then in France in the fall of 1995. The movie isn't part of the story's continuity, even though it marks Gogeta's first appearance.
Goku is overwhelmed by Super Janemba; Vegeta, who has gotten his body back, arrives in time to help but at first refuses to fuse: he'd rather die than fuse with a low-class Saiyan. Goku calmly points out that they're both already dead.
The first attempt fails: Vegeta doesn't extend his index finger properly on the final pose, and the result is a much weaker fused warrior, Veku. For thirty minutes, Veku clumsily dodges Super Janemba's attacks, stumbling and passing gas. Pikkon steps in at just the right moment and buys time for Goku and Vegeta, who pull off the fusion this time.
In the movie, Gogeta appears directly as a Super Saiyan, with a halo over his head, since Goku and Vegeta are both dead at that point. Confident, cold and nearly silent, he announces Janemba's end and finishes him off with a flurry of blows capped by a rainbow-colored energy sphere: the monster turns back into the young ogre he once was, who runs off in fright.
In Funimation's English dub of the movie, Gogeta also speaks with two voices, those of his two voice actors, Sean Schemmel (Goku) and Christopher Sabat (Vegeta); in Japanese, they are Masako Nozawa and Ryō Horikawa.

In Dragon Ball GT, facing Syn Shenron, who is too strong even for Goku and Vegeta both in Super Saiyan 4, it's Vegeta who suggests the Fusion Dance, which makes Goku laugh, given how reluctant Vegeta usually was about it. The fusion into Super Saiyan 4 Gogeta happens in episode 60 of Dragon Ball GT, which aired in Japan on October 22, 1997.
As a Super Saiyan 4, Gogeta has scarlet hair and oxblood-red fur. To taunt Syn Shenron, he pretends to fire a Kamehameha: this “Bluff Kamehameha” releases nothing but confetti and streamers, like a party popper. When the dragon launches his negative energy ball, Gogeta kicks it up into the sky while infusing it with his positive energy, which purifies it.
He then fires the Big Bang Kamehameha, which combines Goku's Kamehameha with Vegeta's Big Bang Attack: under the blow, Syn Shenron spits out all the Dragon Balls except the One-Star Dragon Ball. But just as he's about to fire a second shot, the fusion breaks: Goku is surprised that not even ten minutes have passed. In the end, it's a Spirit Bomb fueled by the energy of the entire universe that lets Goku take down Syn Shenron.

2018: Gogeta enters Dragon Ball Super: Broly
Gogeta only enters the official story with the movie Dragon Ball Super: Broly, released in Japan on December 14, 2018 and in France on March 13, 2019: Akira Toriyama reworks the character in it.
To convince Vegeta, Goku points out that the fusion only lasts thirty minutes and that they've already fused with the Potara; when Vegeta still refuses, he plays his trump card: would Vegeta be okay with seeing Bulma die? Two failed attempts come before success: an obese Veku, then a very skinny Veku.
Once formed, the warrior wastes a little time looking for a name, since “Vegito” is already taken by their Potara fusion; he finally settles on “Gogeta.” He fights Broly in his base form, as a Super Saiyan, then as a Super Saiyan Blue. Just as he's about to finish him off, Cheelai, who has swiped the seven Dragon Balls from Frieza, asks the dragon to send Broly back to Planet Vampa.
Gogeta then stops Frieza, in his Golden form, from shooting down the small ship carrying Cheelai and Lemo away from Earth: he grabs his arm. The Dragon Ball Super manga doesn't adapt this movie; it only references it in a few panels of chapter 42, in bonus pages where Super Saiyan Blue Gogeta gets the upper hand on Broly, and in a flashback in chapter 93.
Where Gogeta comes from: a fusion born on the big screen
Gogeta first appears in 1995, in the movie Fusion Reborn: he's drawn by Tadayoshi Yamamuro; the Dragon Ball GT version is drawn by Katsuyoshi Nakatsuru. Toriyama doesn't take on the character until 2018, when he writes the screenplay for Dragon Ball Super: Broly.
In Akira Toriyama's manga, when Goku and Vegeta are trapped inside Buu's body, Goku does suggest the Fusion Dance to defeat the monster, but Vegeta refuses, wanting neither to strike the poses nor to fuse.
In Funimation's English dub, Gogeta also keeps his two voices in Dragon Ball Super: Broly: Sean Schemmel for Goku and Christopher Sabat for Vegeta, the same duo that shares the role in the movie Fusion Reborn and in Dragon Ball GT. Depending on the version, Goku's voice may have been the dominant one for Gogeta in early productions and in video games, to set him apart from Vegito, whose dominant voice was Vegeta's.
